If a student needs to be temporarily or more extensively exempted from participating in PE activities for medical reasons, families should have the above “Dispense Form” completed by a doctor and return it to the school.
Please note that a medical exemption from physical activity does not automatically exempt a student from attending PE lessons. Wherever possible, students are still expected to attend and participate according to their abilities. The PE teacher will adapt the lesson or provide an alternative role so that the student can remain involved safely and in accordance with the medical recommendations.
Should a student’s medical condition prevent them from making their way to the PE facilities and therefore from attending the lesson, this must be specifically indicated by the doctor on the medical documentation provided to the school.
