One incident can teach something useful without deserving its own permanent checklist item. The learning problem is deciding which findings are local, which repeat, and which are stable enough to become an operating standard.

I use this layer after cases, incidents, and stress tests to compress repeated findings into reusable categories and review standards. Pathway Lens should become more precise through learning, not larger through unlimited incident-specific detail.
Boundary: Operational standards are downstream control artifacts, not general explanatory patterns. This page converts sufficiently supported pathway findings into review categories and execution standards; it does not validate or replace entries in the Cross-Case Pattern Library.
A single incident should not create a new ad hoc checklist for every detail. That would make the review method unscalable and turn operations into compliance noise.
Instead, each new incident should answer a stronger question:
